I, the above named artist certify that all products I offer for sale at the NW Holiday Arts Gift Show are handmade by myself within the State of Washington. I acknowledge that all items presented for jury are representative of work to be exhibited in my display, on above date.
Hold Harmless Agreement-
NW Holiday Arts Gift Show 2010 and its sponsors, organizers, hosts, and beneficiaries shall be held harmless of all liability including but not limited to theft, weather, injury, business conditions, or economic climate.
